Polycare welcomes GatewayMD patients. We've worked closely with GatewayMD to make this transition simple — so the care management program and Hero device support you count on continues.

What's staying the same

• Your Hero device. Your Hero device service continues uninterrupted.
• Your care manager. Once you begin your care with Polycare, your current care manager will continue supporting you in the programs.
• Your other doctors. Polycare works alongside your existing physicians — we don't replace your primary care doctor or specialists.

What happens next

Your care manager can help you schedule an intake appointment with Polycare. It's a virtual visit with Dr. Brian Kohen, a board-certified physician — similar to the initial visit you completed with Gateway. He'll review your conditions and medications, verify your eligibility, and, if appropriate, establish you as a patient. You'll complete new enrollment and consent to the care program with Polycare at that time.

Participation is voluntary. You may also choose to discontinue your care program or review other available providers in the Hero provider directory.
